Summer Well Child Visits and School Forms: What Parents Need to Know

As summer approaches, we would like to share a few important reminders to help your child stay healthy and prepared for the upcoming school year, daycare enrollment, sports season, and summer camps.

1) Schedule Summer Well Checks Early

Summer is one of the busiest times of year for pediatric appointments. Annual well visits are often required for:

  • School enrollment
  • Daycare and preschool
  • Sports participation
  • Summer camps
  • Medication management and annual screenings

We encourage families to schedule well checks as early as possible to avoid limited appointment availability later in the summer.

2) Medical Forms for School, Daycare & Camp

Many schools, camps, and childcare facilities require completed medical forms prior to attendance. Please keep in mind:

  • Forms can only be completed if your child’s annual well visit is up to date.
  • Please allow adequate processing time for all paperwork requests (2 weeks standard turn-around or 48 hours for expedited forms).
  • Some forms may require additional parent information before completion. Please include that information with your medical forms.
  • Submitting forms early helps us ensure they are completed accurately and returned on time. For your convenience, you can email your forms to your office’s forms email: Towson office patients should use formstowson@mainstreetpeds.com. Foundry Row patients should use formsfoundry@mainstreetpeds.com. Please note that we cannot email forms back to you as this is protected health information.

3) Mobile Check-In & Arrival Reminders

To help appointments run smoothly and reduce wait times, our office uses mobile check-in prior to visits.

Please complete the mobile check-in information sent to you before your appointment. If you are unable to complete it ahead of time, please arrive at least 15 minutes before your scheduled appointment time to complete the required information in the office.

Please note:

  • Patients are not considered checked in until they are physically present in the office and all required check-in information has been completed.
  • Delays in completing check-in may impact appointment timing and could require rescheduling if significant delays occur.
